i think the answer is c. because fever is a sign of infection as well as presence of pus..
I think C is the correct answer, because B & D do not corresponds to the nursing diagnosis itself and a heart rate of 88 bpm is considered normal.
The answer for this question is letter C because elevated temperature in this case signifies infection which correspond to the nursing dx of impaired skin integrity r/ to purulent wound drainage.
